At your discretion, all adult family members planning to participate in your care and to help you make decisions are welcome to attend your appointments.
In accordance with the HIPAA Privacy Rule and our office policy, we require you to sign a Confidential Communication Preference form and list those persons authorized to receive information regarding your care.

Once your first appointment has been scheduled, a welcome packet will be mailed to you detailing the necessary documentation to bring to your appointment. As you check in for subsequent appointments, please make our receptionist aware of any recent laboratory or radiological studies you have completed. This will help to ensure that your medical chart is kept as accurate and updated as possible. Additionally, please let us know of any changes to your address, phone number, workplace, or insurance coverage.

Upon your first visit, you can expect to be in our office for approximately 45 minutes to one hour. Follow-up appointments will be significantly shorter.
Our office is open Monday through Friday from 9:00 a.m. – 12:30 p.m. and from 2:00 p.m. – 5:00 p.m., with phone calls answered during these hours.
If you have a medical concern or problem that cannot wait until your next visit, please call our office at (925) 939-9610. If you require attention, we will work you into our schedule that day.
An answering service will answer your call after hours and on weekends and holidays. The answering service personnel will page the on-call physician, who will contact you as soon as possible. Rest assured that one of our physicians is on call at all times.

Due to the many insurance plans available and their continual policy changes, we request that you become familiar with your personal coverage plan and with the benefits, services, and financial responsibilities of your health care.
All non-emergency authorizations and referrals will be mailed to you so that you may personally make these appointments to best accommodate your schedule.
